Decision Tree: Selecting the Appropriate Formula

Healthy term infant → Exclusive breastfeeding possible? Yes → breastfeed. No → Standard cow-milk formula.

  1. If GI discomfort → consider A2 β-casein formula (e.g., a2 Platinum®).
  2. If cultural/ethical preference → consider goat-milk or organic cow-milk.
  3. If suspected CMPA → clinician pathway → extensively hydrolysed → amino-acid formula.
  4. If suspected CMPA- Set 3-hour alerts to track prep-to-discard window.

Deep-Dive: A2 β-Casein Benefits & Limits

  1. Mechanism – A2 protein lacks histidine 67 → no BCM-7 peptide release → potentially gentler on gut.1
  2. Evidence – RCTs show softer stools & less discomfort vs conventional formula.2
  3. Limits – Not suitable for diagnosed CMPA; still contains cow-milk protein.

“A2 formula is a valid first-line option for non-specific GI discomfort, but allergic infants need specialist assessment.”

Prof Sarah Lin, Paediatric Allergist

Switching Protocol: Safety Steps Recap

  1. Consult HCP – Confirm diagnosis & formula type.
  2. Check label & batch – Verify expiry & QR.
  3. Gradual transition – 7-day mixing schedule (see Transitioning Between Formulas guide).
  4. Monitor symptoms – Log stool, rash, sleep for 14 days.
  5. Follow-up – Review with GP/dietitian; adjust volumes.
